Financial / Insurance Information
At ProFormance Physical Therapy, we make every effort to make sure our services are covered by your insurance. We will call your insurance the day of your first appointment to ensure that physical therapy services are covered and your specific benefits. We will inform you of your benefits and your financial responsibilities.
If a co-pay is required, this must be paid at the time of service. Any remaining balance, such a deductible or co-insurance, will be processed after payment from your insurance. We will send you a statement at the end of your 30 day billing cycle, and payment is due within 30 days.
If pre-authorization is required, we make every effort to obtain authorization prior to your first visit.
Keep in mind that knowledge of your personal health insurance policy coverage is ultimately your responsibility. If you have questions or concerns about insurance coverage, please contact your insurance prior to your first appointment.
As a courtesy to all of our patients, we will submit claims to your primary and secondary insurance carriers.
We are contracted with most major medical insurance companies and are continually in the process of obtaining authorization to become preferred providers for other insurances. Please call us to find out if we take your insurance. If we are not contracted with your health insurance company. we will facilitate the authorization process that will enable us to help you with your recovery.
No insurance coverage?
Our goal is to provide quality care to our patients without worry. Because of this, we offer a varitety of payment options for those patients that are unable to utilize health insurance coverage.
For our patient's convenience, we take cash, check, and credit card (Visa and MasterCard).
